H.O.M.E. has established agreements with area agencies and any request for assistance must come through them.  Please contact your case manager if you are in need of assistance.

Anyone receiving assistance, must acknowledge the following prior to receiving assistance.

• An individual/ family can only receive assistance once.

• Anyone receiving assistance must provide their own transportation

• If an individual does not make their appointment, they are only allowed one reschedule.

• Assistance is based on the number of people in the household

• Individual/ family receiving assistance must provide proof that their name is associated with the new residence (lease/ utility agreement).
